Microsoft and Persistent Systems Introduce Population Health Management Solution
In partnership with Microsoft, Persistent Systems, a leader in global digital engineering and enterprise modernization, unveiled a cutting-edge population health management (PHM) solution powered by generative artificial intelligence.
The Solution, which is in line with value-based care models, uses the Social Determinants of Health (SDoH) to ascertain the non-clinical demands of patients and to more accurately forecast the costs of care that are driven by clinical disorders.
“Addressing the causes of negative healthcare outcomes is increasingly understood as a critical component of any PHM program. With this innovative Solution, built on Persistent’s deep domain expertise and differentiated frameworks along with Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights, we are identifying care access and health equity issues by optimizing enterprise capacities and ensuring cost-effectiveness”, said Ganesh Nathella, Senior Vice President & General Manager – Global Lead, HLS Business, Persistent.
“We’re confident that through actionable and trackable insights and personalized intervention, this Solution will significantly improve physician’s and caregiver’s decision-making to enhance patient experience and overall care quality”, added Nathella.
It maximizes capacity and cost-effectiveness for healthcare organizations and providers while assisting patients in receiving high-quality care at the appropriate time and location.
“The goal of population health management is to improve overall health of a population by applying cost-efficient interventions designed to promote better health for groups of individuals. To accomplish this, we need to harness data to identify individual needs, monitor the success of interventions, and benchmark against comparative data”, said David Rhew, M.D., Global Chief Medical Officer & Vice President of Healthcare, Microsoft.
“Leveraging Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights to extract and track crucial SDoH data, Persistent PHM Solution will yield personalized care recommendations and help reduce costs, ultimately contributing to enhanced care quality”, said Rhew.
Based on data from Electronic Health Records (EHRs), Persistent's PHM Solution determines SDoH by utilizing Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights.
Unlike PHM solutions on the market, which rely exclusively on survey data or third-party data sources, Persistent's Solution reliably extracts the critical patient information found in unstructured clinical notes, which comprise up to 80 percent of the patient record within an EHR.
Furthermore, Persistent's PHM Solution helps a doctor or caregiver suggest tailored interventions through community-based initiatives and care management teams to stop unnecessary ER visits, prolonged hospital stays, and readmissions. Up to 60 percent fewer 30-day readmissions are experienced with The Solution, which also constantly tracks outcomes and correlations to guarantee high-quality patient involvement.

“Leveraging Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights to extract and track crucial SDoH data, Persistent PHM Solution will yield personalized care recommendations and help reduce costs, ultimately contributing to enhanced care quality”, said Rhew.
