Microsoft and Persistent Systems Introduce Population Health Management Solution

The Solution, which is in line with value-based care models, uses the Social Determinants of Health (SDoH) to ascertain the non-clinical demands of patients and to more accurately forecast the costs of care that are driven by clinical disorders.
